TherapyPortal can be used by patients to fill out paperwork and schedule appointments.

QUICK GUIDE

  • Contact The Fowler Institute and provide your e-mail address and date of birth to setup your TherapyPortal Account 

  • Set your password from the e-mail sent to you from TherapyPortal.com

  • Fill out our paperwork by downloading and filling out PDF fillable documents (you will not be able to schedule an appointment until your paperwork is filled out)

NOTE: If you fill out the paperwork in a web browser, you will have to Print to PDF when you are finished. See step-by-step instructions for details

  • Schedule or cancel appointments using the Appointments tab

  • You cannot use the original link you used to make your account to go back to TherapyPortal. Instead, navigate to the contact section of our website for the link or click the button at the top of the page that says “Portal Login”
